Surrogacy – Getting started
Although the surrogate isn’t insured under your plan (as a spouse or dependent),1 Desjardins Insurance may reimburse certain eligible expenses you incur on behalf of your surrogate during the pregnancy.
Eligibility
To be eligible, surrogacy expenses must be incurred in Canada by a surrogate who meets all the following conditions:
1. The surrogate must be at least 21 years old.
2. The surrogate must reside in Canada.
3. The surrogate must not be a plan member or plan member’s dependent.
Declaring a surrogate and filing a claim
Declaring a surrogate
Before you can make a claim, you need to declare your surrogate. Here’s how:
1. Go to desjardins.com/planmember and log in to your account.*
2. Under Tools and resources, select Forms.
3. In the MEDICAL AND DENTAL section, choose the form Surrogacy – Declaration – 25018E.
4. Fill out all the sections on the form.
5. Follow the instructions on the form to send it back to us.

Filing a claim
1. Go to desjardins.com/planmember or the Omni mobile app and log in to your account.
2. On the website, under Submit a claim, select Online claim or in the Omni mobile app, select File a claim. Select Start.
3. On the next screen, select Other medical expenses and then Continue.
4. You’ll need to make the claim under the name of the main plan member. Choose their name from the list and then select Continue.
5. For the Type of expense, select Other. Provide all the information requested, including the expense details and the amount claimed. Important: You must provide the required documents for us to process your claim.
6. On the Review page, read over your claim and then select Confirm.
